S7. Franchisee Agreement Info Form

First, have you set up your business entity, if you plan to do so?
1) Meet with an accountant and/or lawyer to determine your business structure and its name. The business cannot be named ‘Gideon’ or ‘Gideon Math & Reading’. Name it anything else. You will receive payments as ‘Gideon Math & Reading – [Location Name]’ using a ‘Doing Business As (DBA)’ filing (see later). 2) Register your business entity with state as needed.
Center Director (Person Who Will Sign Franchise Agreement) Information
Center Director - CD: The individual who owns and operates the Franchised Business and will be physically present and directly supervising at the Center 75% of the time that the Center is open for class. ----If Franchisee is a business entity, Center Director shall mean a member of the business entity who has a controlling interest in the entity, will complete the Gideon Training, and shall be physically present and directly supervising at the Center 75% of the time that the Center is open for class.
